Forest plot of hazard ratios

Is there a user-written Stata program to do a forest plot of hazard ratios after multivariate Cox regression analysis?

if you enter - search forest-, you will find several programs for forest plots in Stata; at least one of the very brief descriptions explicitly mentions hazard ratios ("fintplot")
