Creating a score

Dear All,
I need your help on this matter.
I have performed a multinomial model for an outcome variable called PRONO( which is 3 different medical prognosis: good, bad, very bad) with three dependent variables ( binary variables).
I have obtained different RRR with PRONO=good as a reference.
How can I construct a score to predict which prognosis the patient will have ?
